Quick Answer
No, you can't get a dog in MW4 DMZ, and level rewards don't include one. Rex is only your companion in the Breaking New Ground tutorial, where you head into enemy territory to retrieve a black box together. Your soldier is later captured and killed, so the promised return for Rex never happens.
Dog, Dog Companion, and Rex refer to the same dog you encounter during the DMZ tutorial.

What happens to the dog in MW4 DMZ?
Rex gets shot during an ambush, and you hide him with the intention of picking him up on the way back, but your soldier dies before returning.
Two North Korean soldiers ambush you during the black box operation. Rex kills both attackers, getting wounded in the process. You check on him and drag him into cover so you can retrieve him later.
That return never happens: your soldier is captured and shot in the head. Rex is left at the hiding spot.
Why is the DMZ dog called Rex or Dog Companion?
Rex is the dog's name; “dog” and “Dog Companion” describe the animal and his role beside you in the tutorial.
- Dog: what you first encounter in the opening mission.
- Dog Companion: his role accompanying your soldier on the black box operation.
- Rex: the name used when addressing him during the mission.
